Basically a hard object is in, and blocking, a duct that is too narrow for it to pass through easily, and fluid of one kind or another that is being secreted in whichever bladder and normally goes out through the duct is building up pressure behind it.
This stretches the bladder, and the duct, which hurts.
Sometimes if the rock is relatively small it can be forced through and relief is spontaneous.
But often it can't and surgery (or Lithotripsy) is the solution to remove it before the bladder is damaged by the stretching.
Then removing any others that may be lurking in the bladder is desirable to prevent recurrence.
Trying to find out what is causing the stone formation and stop more developing is also part of good management.
The childbirth analogy has parallels, except that the Uterus is much stronger and can resist the back pressure when it starts contracting and the duct can stretch enough to pass the object in most cases.
However it is a larger, if softer, "rock".
