No idea of the history of it but showing the bottom of your feet (with or without shoes) is one of the biggest insults over there. My guide always gently reminded me about it whenever we were invited to join locals for refreshments.
Don't know if you watched it on tv at the time but when the Yanks put on that performance over there of pulling down the Saddam statue most of the locals started hitting it with shoes.

I reckon is he aimed a bit lower, say at the guts region, he would have connected. He would have needed to side step and perhaps may have been caught in two minds about whether he could duck or step, then as he recovers from the first, the second shoe could have sconned him between the eyes.
