The term "external blow" was used for essentially being hit bat the ball (either batting or fielding at say bat pad). The sort of injuries that can't be faked like a tweaked hammy.
Driving your shoulder into the turf is one way to get time off the field.

I don't have a huge issue with Kohli being allowed to bat normally, as in this instance it is obviously he is actually injured (he'd be out there if he could be, as they are rudderless without him) however it does start to open a can of worms. What if I dive at 2nd slip and pretend I'm hurt, so I can go off and put my feet up for half an hour before I have to bat?

I'm much rather him off the field and Rahane leading them.
If they said field or bat 7, he'd be fielding and we'd be having a tougher time of it with the bat.
