It may be due to the crowd noise; not being able to hear the call to go when one takes off too quickly, but I would have thought that that would have been covered too. Raelene Boyle probably gave the best explanation about it.

I recall in a relay race I was involved in that I went too early, it was because I heard a 'go' call, but it was in another lane. Calling out your countries name or some unique word would sort those problems out.

To be honest by the time you reach Olympic standards you really ought to have that kind of thing sorted out.
